It was the peak season of dengue in 2015. The hospital was overflowing with dengue season in that year there was an outbreak of flu as well. We had many patients with dengue who had platelet counts of around 1 lakh. But we had a kid 7 yrs old who had a fever for 4 days but presented with severe tiredness, poor oral intake and reduced urine output. On further examining the patient the BP was low and the pulse was suboptimal the kid had rashes over the legs and abdomen suspecting dengue we sent for serology and CBC with urine analysis. Meanwhile the patient was admitted in the ward and started on IV fluids. The reports showed hemoconcentration along with a platelet count of around 55,000. The urine showed RBCs. We shifted the patient to ICU and continued with fluid replacement. The fever subsided the next day but the counts reduced to around 20,000 with petechial rashes the patient was started on oral supplements for thrombocytopenia and 2 units of FFP was transfused according to the patient's weight. The patient's count was repeated the next day which showed gradual improvement to nearly 30,000. The kids' general condition and Vitals improved, the patient continued on the same course of treatment and the counts further increased to 70,000. The patient was shifted out of ICU and discharged the next day. The patient was advised to repeat the count after 2 days and to continue the oral supplements. The repeat counts increased to 2 lakhs. It is always easier to prevent rather than treat dengue.
