In 2017 we had a patient presenting with high grade fever for 6 days with severe body pain. He was on medication taken OTC from a nearby Pharmacy. He came with H/O blood in urine for 1 day. On examination the patient showed signs of moderate dehydration with poor vitals his peripheries were cold and clammy. We immediately started him on IV fluid bolus and sent for the basic investigations including serology for dengue and malaria. The patient’s investigations revealed severe thrombocytopenia of counts 15,000 and bleeding manifestations, on shifting the patient to ICU he was given 4 units of FFP and 1 unit of single donor platelet. The count repeated on the next day didn't show any improvement it was around 12,000 and the patient’s vitals though better didn't show any drastic improvement and he continued to have bleeding manifestations and hence was given transfusion the second day too with 4 units of FFPs and 1 unit of single donor platelet. The counts repeated the next day showed moderate improvement to 16,000 with no further episodes of hematuria, the vitals also improved. The same course of management was continued for 2 days and the patients' counts were monitored daily. On the 4th day the count increased to 1.3 lakhs and was discharged.
